What will you be doing? The Care Coordinator will be responsible for the overall management and scheduling of client rotas to ensure that the care delivered is of the highest standard. You will work with all internal teams to facilitate patient centred care that enriches the quality of our client’s lives and enables them to live within their own homes whatever their abilities. Responsibilities of contact for clients/families and all staff relationships with all clients, families, and staff to create and maintain trust in all aspects of their packages. all shifts for the packages they support and Responsible for covering missed shifts/annual leave & sickness with bank staff and or Agency support. in partnership with the Clinical Leads to maintain the ongoing safety of the service users we support. Form interviewing, to group meetings and appraisals. supervisions for all staff to track their happiness and well being. for capturing any client and carer feedback forms to help develop the service we offer to all. multi-disciplinary meetings with other professionals, including commissioners and medical professionals that help support the service users we look after on a day to day basis.
