The Fracture Liaison Service monitors the pathways for patients at risk of fracture or of further fracture. The post holder has a role in enabling the Trust to deliver a high standard of care and communication throughout the fragility fracture patient’s pathway. Effective tracking of the pathways facilitates the Trust in achieving CCG Commissioning Parameters, NICE recommendations, and national standards of care for patients with fragility fracture. The post holder will work closely with the Fracture Liaison Nurse Specialists and other members of the Hospital’s Administration Team. Ensure patient details are accurately updated on CareFlow and within medical notes as per University Hospital’s Bristol and Weston (UHBW) protocols. Manage referrals into the service and enter them onto the appropriate CareFlow waiting list. Process Bone Health Assessments forms from the fracture clinic and send questionnaires to those patients not processed in clinic. Initiate the follow up of patients at 4 and 12 months post fracture and update the FLS tracker as necessary. Maintain a stock of supporting literature and patient booklets for patients supported by the service. Generate letters to patients, GPs and other health care professionals involved in the care of the patients using the recognised UHBW systems including Digital Noting, Synertec and clinical document storage (CDS) systems. Manage the FLS group email inbox. Manage the FLS tracker. General administration e.g. scanning documents onto Evolve. Collecting and distributing post.
