Undertake specific skilled support to palliative care patients referred to the Therapy Team in both In-patient and Community settings. Assess and treat patients both individually and in groups as required in a variety of settings including inpatients, outpatients, in the community and Living Well Programme. Be able to monitor and progress patient treatment as necessary within established guidelines, referring back to the therapy team when goals have been achieved. Carry out administrative and household tasks as necessary to maintain the smooth running of the therapy and support team. Monitor and order stock, mobility aids and supplies for the therapy and support team. About us Undertake specific skilled support to palliative care patients referred to the Therapy and Support team in both in-patient and community setting Independently perform basic assessment and treatment of patients functional problems both individually and in groups as required in a variety of settings including inpatients, outpatients, in the community and Living Well Programme. Lead group sessions under the supervision of a therapist for the Living Well project both face to face and online. Manage a small caseload of outpatients under the supervision of a therapist. Assist in the development of junior members of the therapy team by being actively involved in in service training. To be able to triage referrals to the appropriate members of the therapy team. Assist in teaching and promoting therapy core values within the hospice and community settings.
