We are seeking a pharmacist who is already an independent prescriber or someone interested in becoming a prescriber. The successful candidate will work as part of a multi-disciplinary team to provide medication optimisation and prescribing support involving patients facing activities such as structured medication reviews, chronic disease management, safer prescribing initiatives. You will have excellent communication skills and wish to work in a clinical setting delivering pharmaceutical care to our diverse patient population. This post requires autonomous working to provide pharmaceutical care to our PCN population. You will have daily appointments to offer medication reviews and medication advice with protected time to conduct structured medication reviews and other contractual obligations, including QOF and IIF.
