This position is open to physiotherapists and occupational therapists with experience of working in the community and who wish to develop their assessment, care planning and reablement skills whilst being part of a dynamic, integrated, multiagency, multidisciplinary team. You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic goals. You will need excellent organisational skills along with a wide knowledge base and experience of working in older person's services. We will offer a working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system. We are based in Hayle and cover Penwith including Hayle, Penzance, St Ives, Marazion, Land's End and surrounding areas. This is a 28 hour post and days are planned over a 7 day rota which involves regular weekend working and some bank holidays: the successful candidate must be flexible with their working days. We cover 8am-6pm. You will need to be able to a triage, assess and provide evidence based treatment plans for adults within the catchment area, either in their own homes or nursing and care homes. To work alongside the wider multidisciplinary teams within the community, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services.
