The service is also an early implementer site for maternity mental health services; broadening the remit and profile of specialist interventions available for women and ensuring an enhanced care pathway. We are looking for an experienced mental health nurse or practitioner (social worker or occupational therapist) with an interest in perinatal mental health to join our team for 12 months to cover maternity leave. The role of the perinatal community mental health nurse/practitioner will be to undertake comprehensive assessments, manage a complex caseload of service users, provide practical advice and support including risk management, psychological interventions and overseeing any safeguarding issues. Care coordination and care planning within the Multidisciplinary Team, as well as liaison with external stakeholders is a crucial part of this job to ensure patient led, collaborative, high quality care is being delivered.
