The Post holder will work as part of the Cardiology team to provide advanced clinical assessment, support and onward referral for patients attending the Emergency Department, Emergency Assessment Unit, and any department or ward in the OUH sites and undertake to assess patients presenting in the Rapid Access Chest Pain clinic The aim of the post holder is to expedite assessment, including appropriate tests and investigation based on clinical need and to provide quality of care and support for patients and their families. The Post holder will liaise across organisational boundaries with the health care teams in primary care, secondary care and tertiary care to establish systems of care that support the referral and management of patients that have or require urgent admission with a range of Cardiology conditions About us The Post holder will work as part of the Cardiology team to provide advanced clinical assessment, support and onward referral for patients attending the Emergency Department, Emergency Assessment Unit, and any department or ward in the OUH sites Undertake to assess patients presenting in the Rapid Access Chest Pain or Outpatient cardiology clinics within the Oxford University Hospitals NHS Trust with cardiology conditions. The Post holder will provide and facilitate the delivery of multidisciplinary care for cardiac patients at the OUH. The post holder will contribute to the development of cardiac services at a local and national level, providing nursing expertise for patients, families and carers. The Post holder will support and guide activities; developing and motivating staff education in order to ensure the development of effective, evidence based multidisciplinary practice. The Post holder is responsible for providing expert practice through leadership, consultation, clinical advice, teaching and support for staff. The post holder will work with clinical colleagues to provide information, advice and support to patients with complex cardiac conditions. The Post holder will respond to changing needs within the area of cardiology, taking a professional lead in the development of an evidence based approach to practice, fostering a culture that contributes to the clinical governance agenda.
