The multi-disciplinary team, work with adult patients in their own homes, Community Hospital, or care homes to promote independence and improve quality of life by supporting them to remain at home or be discharged home. Therefore, being a car driver is essential for this role. As part of this role, you will be required to work as member of the therapy team at Swanage Community Hospital. You will be joining a motivated, caring, inclusive and compassionate team. You will work with a team of Nurses and Therapists to set goals and assist patients to regain their independence.We offer opportunities for further learning and development, including the chance to apply for Nursing, Physiotherapy and Occupational Therapy Apprenticeships. Responsibilities will require you to work mainly under the close supervision of the clinicians within the team, but at times also to work autonomously. Previous rehabilitation/care experience, working in the community or on a ward would be desirable.
