To support the Acute Oncology Team Leader and patient flow managers in managing the flow of patients through the trust and moving the bed management process forward. To work with the multi-disciplinary team to provide real time information to patient flow managers regarding bed status. Facilitate flow of patients through the hospital by transporting patients from one care setting to another with direction from registered nurses Support care settings in preparing for admissions by preparing bed spaces, communicating with nursing teams. Facilitate transfers of patients from one care setting to another by taking/giving handover from one team to another, escorting patients who require nursing escort, carrying out clinical tasks to prepare patients for transfer and liaising with relatives regarding transfers. Assist staff in care settings with clinical tasks to allow them to safely accept transfers. Orientate patients to a new care setting upon arrival and assist staff with admission process where necessary. Co-ordinate the transfer lounge including taking responsibility for care of patients in the lounge. Managing admissions to lounge including taking handover and assessing patient suitability for the lounge. About us DUTIES AND RESPONSIBILITIES 1. Liaise closely with all Ward Managers/Nursing Teams and Matrons to ensure accurate up-to-date knowledge of patient placements is achieved. 4. To work closely with the infection control team. Inform them of any high-risk patients admitted to the hospital, and to facilitate the movement of any newly diagnosed high-risk inpatients within the hospital to the correct location 5. Liaise with other hospital Trusts to appropriately repatriate patients from other Trust sites and manage the requests for inter hospital transfers working closely with medical teams to identify clinical priority. 6. To co-ordinate and where necessary take responsibility for care of patients in the transfer lounge 7. Work with and where necessary cross cover the AOMS senior HCAs to support the Hotline Team who provide a 24hours help and advice line to the trust to provide a safe admission process for emergency admissions TRAINING & EDUCATION 1. Complete appropriate clinical skills and mandatory training as directed by management 2. Participate and undertake research and audit which will influence practice 3. To be up to date and competent in all appropriate areas of clinical practice including medicines management, moving and handling, assessment of critically ill patient and competent in performing ECGs 4. Attend all mandatory study days 5. Maintain awareness and competency within the role against national and regional initiatives and subsequently implement these initiatives into practice. PROFESSIONAL 1. Act as a resource giving advice and support to nursing staff regarding emergency admissions and associated issues 2.
