These are community-based roles that require frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements. Band 6 Memory Assessment Nurse As a Memory Assessment Nurse, you will deliver high-quality memory assessments, contributing to the early diagnosis of dementia and supporting patients and families through the diagnostic journey. You will manage a community caseload, work collaboratively within a multi-disciplinary team, and provide person-centred care to enhance the lives of those living with dementia. Band 5 Primary Care Dementia Practitioner (PCDP) In this role, you will be a key link between primary care and mental health services, supporting patients and their families from diagnosis through ongoing care. You will manage a community caseload, provide education and advice to caregivers, and work closely with primary care professionals to ensure integrated and effective support.
