An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated and forward thinking practitioner to join a busy supportive team based in the Denbighshire area. The successful candidate whilst working closely with a multi - disciplinary team, will receive referrals directly from GP's and provide short term interventions for individuals with mild to moderate illness and will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies. The successful candidates will manage a caseload of patients with whom they will work alongside to formulate risk management plans and care planning based on robust assessment of need. We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.
