The successful candidate must be committed to working clinically in planned care delivery, empowering patients with a diagnosis of Heart Failure to maintain a high quality of life whilst monitoring symptoms. You will join our Specialist Nurse led team, who form part of MPFT Long Term Conditions Specialist services community care provision in Staffordshire, working in partnership with both the local hospitals and wider primary care services across the regional area. Essential elements of the role include review and clinical monitoring of cardiac treatment and medication plans put in place for patients by the specialist nurses. Providing continued planning and implementation of holistic packages of care with the MDT, to support and enable Heart failure patients and their carer's to live the best quality of life as possible in the community setting. Our skilled specialist nurses are autonomous non-medical prescribers, and working alongside them you will provide community domiciliary care visit support, monitoring patients who are undergoing rapid cardiac titration as part of optimisation of evidence based cardiac therapies. The main aim of the Heart Failure nursing role is to provide an intense level of care to optimise therapies, support the individual to manage their condition, prevent unnecessary admissions to hospital, and to improve the provision of palliative care for patients with this long-term condition. You will gain focused skills to support cardiac service delivery to patients and provide further holistic assessment intervention and support. You will need to be a NMC registered nurse who has a real interest in the nursing management and support of patients with Heart failure.From initial diagnosis to end of life provision and specialist support. You must have a desire to work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes. You will need experience of nursing patients with long term conditions, have good core clinical observation skills, venepuncture skills and the potential to deliver IV therapy to patients in the home.If About us Key Responsibilities Provide an optimal level of care for patients with heart failure, by demonstrating an understanding of the underlying pathophysiology of heart failure, mechanisms of therapeutic intervention to the condition, and the significance of physical and psychological changes within the patient. Responsible for reviewing, implementing and evaluating programmes of care, which will support the patient by providing an equitable, efficient, effective high quality and safe heart failure service. Build upon existing partnerships with specialist palliative care services to facilitate the delivery of a palliative care approach to patients with advanced congestive cardiac failure Assist in the delivery of seamless care through the use of integrated care pathways in collaboration with GP's, Consultants, Palliative Care providers. Undertake an appraisal with line manager annually Assist with the collection, analysis and evaluation of data relating to the heart failure service, which includes patient related outcome measures. Maintain personal & professional development in line with the NMC \"Code of Professional Conduct\" and understand accountability within the \"Scope of Professional Practice\".
