Key deliverables include work to facilitate a smooth patient journey ensuring that co-ordinated and streamlined administrative processes revolve around patients identified as eligible for cancer surveillance following a screening fibro scan in Liver Health Checks clinics across the network. The Pathway Navigator will support the patient journey by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs. The post holder will have administrative responsibilities including data management and patient tracking for those individuals participating in the liver health checks programme including patients referred into hospital-based liver cancer surveillance clinics. The purpose of this role is to complement and support the existing speciality team to support patients from referral, through diagnosis and onto treatment and beyond by providing a safe, seamless, patient centred services. Maximising the health and quality of life of the patient and carers and improving the quality and efficiency of health care delivery. About us Being responsible and accountable for their practice and behaviour underthe guidance and with the support of a registered practitioner and byworking as part of the speciality multidisciplinary team. Working with primary care colleagues and understand liversurveillance pathways in order to support patients to attend theirappointments and investigations, providing a point of contact andpromoting relevant and timely communication. Working in conjunction with the speciality team to provide coordination ofhigh-quality patient care through on-going telephone/ face to faceassessment and proactive identification of needs using basic knowledge,approved tools and procedures. Be the single point of contact for all patients under the care of the speciality team including those patients on person centred follow-up. Coordinate care, track and update on the results for non-complex cases, managing patient concerns via the helpline and generic email, facilitate rapid re-entry into the system if required. Work with the clinical team to coordinate any necessary assessments,appointments, investigations and organisation of MDTs, virtual MDTS andoutcomes in order to fast track people and provide a seamless patientpathway. Navigating the complex health and social care system to support patientsat all points in the pathway. Building relationships with professionals andfacilitating access to a variety of services including generating referrals onthe persons behalf where necessary. Working as part of the speciality team to facilitate Holistic NeedsAssessments and coordinate care for patients on a HCC surveillancepathway who have non-complex needs so they can access the right helpat the right time, providing a seamless experience of care. Recognise when there is a potential deterioration in condition or anemergency situation and escalate these concerns to a registeredpractitioner in a timely manner. Early detection of problems/ actions toreduce in-patient admissions. Awareness of HCC Liver Surveillance and systems to ensure all mandatorydata is accurate and timely. Documenting and monitoring all aspects ofpatient care coordination and service delivery. Clinical Practice Triage incoming calls and initiate appropriate response according toassessment tools, protocols and individual pathways, liaising with the CNSas appropriate. Make pre planned outbound telephone calls to patients to assess needs andmonitor progress, liaising with the CNS and other members of thehealthcare team as appropriate in order to initiate a proactive preventionapproach. For patients on the pathway ensure that test results, clinic letters and anynotes are available and communicated to the clinical team at the earliestopportunity to inform the next steps of the pathway. Coordinate the necessary assessments, appointments or investigations asidentified in patients care plan. For patients on the pathway, work closely with the MDT coordinators toensure good preparation of the cases for the MDT discussion and ensurethat MDT outcomes are followed up in a timely manner. Maintain the HCC Surveillance database for follow up, monitoring alerts,organising relevant tests and ensuring results are flagged to the relevantMDT member. Support the delivery of patient information to ensure all patients/carersreceive appropriate verbal/written communications on a timely manner. Assist in CNS teams audit of patient satisfaction survey and clinical audits. Evaluate outcomes of care delivery with the registered practitioner.
