Although the holder of this post will support both the Therapy and Nursing teams, the primary focus of this post will be to support the Therapy team. We are a busy dynamic team who actively support staff new to the community. The South Rural Integrated Neighbourhood team is based at Beacon House, 53 White House Road, Ipswich IP1 5PB. You will be required to work across the South Rural area which supports patients from Hadleigh, Bildeston, East Bergholt and Capel, Holbrook and Shotley GP surgeries. There may also be times when you are required to support other community teams to meet service requirements. The team consists of Occupational Therapists, Physiotherapists, District & Community nurses, Assistant practitioners, and Generic workers. We also work closely with our social care colleagues within the INT. The Therapy team is led by a Band 7 Therapy Lead with an INT manager managing all teams within the INT. You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to the post. The role involves providing therapy interventions to patients in the South Rural area. You will work closely with the therapists in the team and follow treatment plans that are set by them. You will also be expected to complete nursing competencies as part of the generic worker role and carry out nursing interventions as required to housebound patients, following care plans as set by the nursing team. Our main aim is to support, maintain and encourage people to make healthy lifestyle choices in the ageing well process. Conditions include long-term neurological conditions, palliative and end of life care; diabetes; acute medical episodes, wound care, and phlebotomy - all with a view to preventing admission and supporting early discharge. Many of our patients have a complexity of needs across the spectrum of physical and mental and social care needs.
