Work autonomously to assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence based practice which improves the quality of care and meets the needs of patients. A holistic approach to caring should be planned that will include the appropriate level of physiological; pathological; spiritual, socio-economical and cultural aspects which influence the care of clients within your specific speciality. To manage a significant caseload of patients with complex diabetes, managing individual programmes of care working with patients for self management of their diabetes. Provide community nurse led clinics within general practice. Work within the MDT diabetes team supporting general practice within virtual review clinics. Work proactively to prevent admission to secondary care Run clinics for patients on insulin pumps in conjunction with the consultant To provide education for patients with type 1 diabetes and type 2 diabetes About us Please note for this role you will be required to travel independently around the county meeting strict time deadlines.
