Acting as a resource for general practice and working to reduce admissions, improve diabetes outcomes and quality of life for people with diabetes. To provide advanced clinical practice in the field of diabetes care and ensure the provision of a quality patient focused diabetes service in Cambridgeshire and Peterborough DSN to be based at the Healthy Living Centre in Peterborough and preferably have experience in managing patients with type 1 diabetes including Young people and those who are using insulin pumps We will need T1 / Pump and possibly also Young Persons experience. Work collaboratively with the team and professional leads to support professional and practice developments including CPFT clinical supervision framework. Work autonomously to assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence based practice which improves the quality of care and meets the needs of patients. A holistic approach to caring should be planned that will include the appropriate level of physiological; pathological; spiritual, socio-economical and cultural aspects which influence the care of clients within your specific speciality. To manage a significant caseload of patients with complex diabetes, managing individual programmes of care working with patients for self management of their diabetes. Provide community nurse led clinics within general practice. Work within the MDT diabetes team supporting general practice within virtual review clinics. Work proactively to prevent admission to secondary care Run clinics for patients on insulin pumps in conjunction with the consultant To provide education for patients with type 1 diabetes and type 2 diabetes About us Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. The post holder must adhere to the Trust Risk Assessment and Risk Management Processes The post holder must adhere to Infection Control Policies and Procedures. It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration. Participate in clinical supervision on a regular basis and be responsible for the management of and reporting on individual and team performance indicators and development of remedial action plans as required, The post holder must also participate in clinical and other audits as required.
