The team covers Greater Preston and Chorley South Ribble area - based at Minerva Health Centre. We consider flexible hours and remote working. You will gain experience in enteral feeding, nutrition support including experience in working with weight management and gastroenterology services. Your role will involve the assessment and implementation of care plans for patients in clinic, in their own homes, in nursing and care homes, possibly at some local community hospitals. You will assist in the training of health care professionals and carers and maintenance of IT systems. You will provide a high standard of assessment, diagnosis, treatment and expert advice to HEF patients and carers. You will have responsibility for the delivery of a high quality clinical service to local and national standards. You will act as a positive role model, leading by example, mentoring and supervising staff and providing opportunities for continuing professional development. You will contribute to achieving the objectives of the clinical service group/speciality objectives and work within the clinical governance framework through the core dimensions of the role, clinical practice, leadership, acting as the patient advocate and through audit and education. Due to the nature of the role, it is essential that you are able to travel independently across the locality and have access to a suitable vehicle for business purposes. The Specialist Community Dietitian will act as the clinical expert and professional resource to the multi- professional team, patients and carers within their Speciality Service. The post holder will have a responsibility to develop the specialist area of practice in partnership with other members of the speciality team will work collaboratively to develop services and improve the quality of care delivered. The post holder will carry responsibility for a defined case load and offer specialist dietetic advice including setting standards of care, contributing to specific care guidelines and supporting the care of patients across the health economy. The post holder will also have an active role in the education of patients, carers and healthcare professionals in their area of expertise.
