In this role, you'll be at the heart of our Older Adults Service, where your passion for supporting individuals with mental health needs, including dementia, will shine through. As part of our Older Adults Community Mental Health Team, you'll provide compassionate mental health care and treatment, following the latest guidelines like the Dementia Living well Strategy and NICE recommendations. You will have the opportunity to work across CMHT(e), HTT and Memory Clinic and you will be responsible for managing your own caseload, working collaboratively with other disciplines from both statutory and voluntary sectors. Work collaboratively as an integral part of a dynamic multi-disciplinary team, managing a diverse caseload of patients with care and compassion. Conduct thorough assessments of patients, ensuring active involvement of their caregivers and family members in developing comprehensive care plans. Take ownership of formulating, implementing, and assessing the effectiveness of care plans for a specific group of patients, including those with complex needs, in close collaboration with patients and the multi-disciplinary team. Offer direct supervision and guidance to Support Workers and/or students, fostering their growth and development within the team. Provide expert advice to team members and external agencies regarding the unique needs of service users, contributing to the overall improvement of service delivery. "}
