This job is for you! Working effectively within the team you will provide evidence based interventions. You will manage a caseload and will be required to undertake assessments and reviews. Using the Care Programme Approach, you will be a care coordinator, communicating effectively with those involved in the care of people in custody up to release. The role will require you to be a reflective practitioner, eager to work collaboratively with individuals, carers and other professionals to contribute to the delivery of an effective, dynamic, integrated mental health and substance misuse service. You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway. You will help to rehabilitate and support service users to live hopeful and fulfilling lives as members of the community. A summary is as follows: Take a Recovery focused and holistic approach in working with the people who access our services. Deliver care and interventions utilising the stepped care model, including provision of the CPA Framework where appropriate. Assess people's needs to develop, implement and evaluate personalised care plans. Facilitate therapeutic group work taking the lead where appropriate. Deliver evidenced based interventions. Identify people requiring hospital treatment and facilitate the timely transfer under The Mental Health Act. Line management of Band 5 nurse where appropriate.
