The focus being on supporting GPs in their day-to-day management of patients, specifically aimed at reducing the administrative burden, making best use of consultation time and supporting those particularly vulnerable to isolation who are regular attenders at the practice. The role will be varied, working very closely with the GPs and includes direct contact with patients and outside agencies. The successful candidate will be reliable, self-motivated and be able to use their initiative to solve problems. We are looking for someone with excellent communication and organisational skills who can prioritise tasks, free up GPs time and contribute to the smooth running of appointments, improving patient experience at the practice. This position is 37.5 hour per week, with hours worked across Monday to Friday Interview to be held on Monday 30th December 2024. Access to NHS discount scheme Cycle to work scheme About us The focus of the role is on supporting general practitioners in their day-to-day management of patients specifically aimed at reducing the administrative burden, making best use of consultation time and supporting those particularly vulnerable to isolation or who are frequent attenders at the Practice. The role will improve patient access and patient flow within surgery hours increasing the time-efficiency pf appointment and reduction of waiting times. Key tasks and responsibilities: To offer a high-quality service to the patients by providing smooth-running efficient support to the general practitioners at BMG. Support patient experience by translating or reiterating information from the GP. Organise and facilitate group consultations/ clinics. To offer a high-quality service to patients by providing smooth-running efficient support. Work to improve access and reduce unnecessary contacts with clinicians. Completion of frequent attenders searches, provide regular contact and continuity with these patients, signposting where appropriate. Monitor and review to improve efficiency and report on outcomes. Identify and have a good understanding of the types of services and facilities which patients may require access to for care support e.g. signposting to social services to access care support, referral to care co-ordinators/social prescribers and know how to escalate if required. Provide post assessment actions to support GP management. The GPA will have responsibility for completion of referral forms such as community health monitoring team, SALT, community therapy as well as NHS digital weight management. Processing T-Quest requests. Completion of 24 hour ECG/ ECHO referrals and booking prerequisite investigations. Obtain test results on behalf of clinician by liaising with hospitals/using Indigo system. Communicate sensitive information, test results, explain medication changes and arrange appropriate follow up with patients. Gather further information from patients and third parties when required, ensuring the information is recorded appropriately in patient records. Support clinicians with completion of medication reviews. Identify patients who may need clinician review, arrange appropriate physical health checks and update GP. Liaising with family following notification of inhouse Child DNAs or receipt of Secondary Care Child not brought letters and reporting back to the GP. Resolve inappropriate requests from hospital departments. Liaise with hospital teams to inform them of results/ensure results are actioned by the hospital requestor. Act as a chaperone in accordance with practice chaperone policy, including record keeping where appropriate. Optimise appointments by running Did not attend searches to identify these patients. Highlight and discuss these patients in Micro team meetings and work alongside patients for improvement. Produce reports to inform Quality Improvement.
