The successful applicant could be a nurse or allied health professional with the required qualification, skills and attributes to fulfil the job description. The successful applicant will work with the multi-disciplinary, consultant practitioner led team to deliver high quality assessments, diagnosis and therapies to patients in their own homes. The team looks after patients that have acute needs, by providing a range of hospital level interventions and diagnostics as an alternative to hospital admission.
