The post-holder is expected to work clinically on the Kensington Wing, supporting the teams with the provision of high quality, seamless and patient-centred care across the antenatal, intrapartum and postnatal care pathways. The Quality, Safety & Improvement (QSI) midwife is an experienced registered midwife who is passionate about improving the safety, and effectiveness of private maternity services. They will be proactive in sharing the learning and themes from national reports and local incidents. They strive to develop practice, informing the training needs for the service and driving the highest standards of care. This role will work alongside the NHS Maternity Quality & Safety team in the implementation of a robust governance framework across the service. Through ensuring high quality internal reporting and communication across the Trust and to external partners. The post holder will work to identify, monitor and support risk issues within the department, and ensure the workforce take a proactive approach to mitigating risk and improving safety. The role will include liaising with families, to ensure the service complies with the duty of candour responsibilities and ensure good communication throughout the investigation process. Organising and supporting debrief meetings following serious incidents where appropriate. Working in conjunction with the senior team and Professional Midwifery Advocates (PMA's) to ensure Serious Incident reports and learning are shared with staff and implementation and embedding of learning is evidenced through relevant audits. Please see attached job description for further details. About us Roles and responsibilities Maintain a high profile in the clinical area and be accessible to all staff. Develop and maintain communication channels, engaging all roles within the service and ensure regular updates are provided. To provide professional and operational support to the Midwifery clinical and management team in relation to Claims, Complaints, Clinical Risk and Governance initiatives and issues. Assist with the day to day maternity risk management process and strategy, Including the accurate reporting and validation of incidents onto the Datix electronic system and to ensure that the appropriate actions are taken in relation to any risks that are identified. To be responsible for ensuring incidents from the service are reported as appropriate, via the Datix reporting system. To co-ordinate, lead or delegate investigations and provide leadership and support for staff when serious incidents (SI) occur within maternity services. Contribute to the notification, and investigation using the PMRT role. To be able to support staff following traumatic cases. Risk register complete risk assessments, as required, and ensure that these are discussed at the appropriate forum and escalated appropriately. Work with external agencies such as HSIB, to ensure timely reporting, uploading of information and communication. Support the maternity clinical dashboard and ensure it is presented at the appropriate forums and externally to commissioners. Support the activities of the weekly Maternity Risk meeting (Tuesday) and monthly Maternity Forum meetings. Be the first point of contact for PMRT for both professionals and families. Ensure the service remains with PMRT timescales, recommendations and service changes. Ensure staff are receiving training and are knowledgeable about PMRT and other external review bodies. Working with Quality and Safety Lead to take responsibility for co-ordination and completion of National and local Quality and Safety audits across the service in liaison with the governance team as required. Ensure learning and outcomes are disseminated across all areas. To act as a clinical role model promoting professional, competent standards, working closely with the multi-disciplinary team. Ensure all communication, which may be complex, contentious or sensitive, is undertaken in a responsive and inclusive manner, focusing on improvement and ways to move forward.
