An Exciting opportunity has arisen for a Colorectal Cancer Care Coordinator. This role is to support the Colorectal Cancer Nurse Specialist Team. This is a secondment position to cover maternity leave for 12 months. We are looking for a motivated individual, with good communication skills, who wants to learn more about the role of cancer care coordinator to support the current team. A Cancer Care Coordinator helps cancer patients navigate the complex health and social care system during and after their cancer treatment. The post holder is an important member of the team. They will contribute to the improvement of patient care and the patient experience by improving access to the service and providing seamless coordination of care. The main duties of a cancer care coordinator are to support the CNS team and the patient. This will involve: Triaging incoming phone calls, providing general advice to patients/carers and referring complex care needs to the CNS. Undertake holistic needs assessments and the development of an individual care plan/ treatment summary under the direction of the CNS. Coordinate appointments or investigations to fast-track patients through the system. Make pre-planned outbound telephone calls to patients to assess ongoing health and social care needs and make the appropriate sign posting suggestions. Encourage and support healthy lifestyle choices. Provide psychological support to patients/ families/ carers. Communicate effectively with all members of the clinical teams/ cancer services and health and social care services. Contribute to data collection, audits and surveys. Organise and prioritise the designated workload. Provide administrative support as required.
