An exciting opportunity has arisen for a permanent Band 4 Patient Pathway Coordinator working for Intermediate Care Lambeth & Southwark. The post holder will work across sites at the Whittington & Queens Road to support both integrated services. This role is 37.5 hours weekly and includes weekend work. This is an excellent opportunity for those looking to develop their experience and skills in administration. We are looking for a highly organised person who is able to prioritise a busy workload in a demanding environment and perform well under pressure. The post holder will process referrals into the services and allocate patient visits. They will also be responsible for the collation of data for the service using relevant databases and clinical systems, and will provide a high standard of professional and patient focused service to a range of Trust-wide specialties' and will support the function on a day-to day operational basis. The successful candidate will have a good understanding of clinical systems, strong communication skills, attention to details and be proactive to achieve desired outcomes. The role requires the ability to liaise with all disciplines of staff i.e. Senior Practitioners, OTs, PTs, Nurses and administrative colleagues. Candidates must be flexible to the needs of the service, attend meetings as required and have excellent communication skills. The post holder will provide a range of administrative support to the multi-disciplinary clinical team, especially working very closely with Clinical colleagues. The Patient Pathway Coordinator will lead on the tracking of pathways of patients, process referrals, record data on internal database/s within agreed timeframes, manage any data requests, collate clinical data onto databases and work closely with the MDT. By working closely with a wide range of clinical and managerial staff, and providing high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met. Guy's and St Thomas' is comprised of four Clinical Groups (CGs) and Integrated and Specialist Medicine is one of the largest of the groups with a budget of over £500m and approximately 6200 WTE staff. The Integrated Local Services directorate within this clinical group provides a range of community and inpatient health services for adults within the communities of Lambeth and Southwark, with some services also provided in Lewisham. Our hospital-based inpatient and outpatient services provide care for adults from across the region.
