T&C's apply. We are looking for a nurse who has experience of working in the community to work within the End of Life Care Team. In this role you will provide care, support to palliative patients within the community setting to provide a seamless service whilst a complete package of care is put in place. The post holder will undertake a range of delegated tasks. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care. You will have excellent organisational and time management skills and be passionate about delivering holistic care that has the patient at its centre. You will relish developing new ways of working to improve quality and develop a "one team" culture.
