                                                    Pharmacist - maternity cover with Primary Care Careers in Dunmow, Essex

                                                     South Uttlesford PCN   is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi-disciplinary team. The role is offered on one years fixed term contract to cover maternity leave on a full-time basis for 37.5 hours per week. This is an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join us working across two sites, Stansted and Angel Lane Surgery.     Managing long-term condition clinics to review the ongoing need for each medication.   Undertaking clinical medication and care home medication reviews.   Providing telephone medicines support.   Assisting with unplanned hospital admissions, review of medications and reconciling of medications.   Repeat prescribing.   Identification of high risk patients.
