This is an exciting opportunity for an experienced, enthusiastic and proactive practitioner, with a keen interest in respiratory care, to join our established community respiratory service, including: pulmonary rehabilitation, respiratory outpatient physiotherapy, home oxygen and respiratory nursing. The successful candidate will work with experienced respiratory clinicians to provide high quality, personalised care to the people of Scarborough, Ryedale and Whitby. You should have experience supporting patients with a range of conditions, such as COPD, bronchiectasis, asthma and pulmonary fibrosis. You should be able to work flexibly, as part of a team, to meet the needs of the service. To thrive in this role, you will need to be passionate about community healthcare provision, able to tackle key priorities with enthusiasm and have the tenacity to deliver patient care in an ever-changing environment. The successful post holder will have a university or college qualification relevant to this role. The post holder will provide support to experienced qualified clinicians to deliver high quality, personalised care for respiratory patients via pulmonary rehabilitation classes, outpatient clinics and home visits. The successful candidate will be instrumental in supporting the specialist clinicians to deliver the community respiratory service by means of assessments, reviews, and exercise classes, working collaboratively with the team to strive for recognised quality standards, adapting programmes of care to meet the individual needs of complex patients. Patient experience and voice will be captured and utilised to drive innovations in user engagement, service equitability and quality improvement. The appointed applicant would be competent in undertaking basic physical or psychological tests, involving observation and assessment skills, recognising and reporting significant findings to a qualified member of the team; also being able to recognise emergency situations and respond accordingly. You should feel able to support patients to improve and maintain their mental, physical and behavioural health and wellbeing through health promotion and with clinical advice surrounding their management. This may also involve identification of additional requirements and referral to other specialist services within the trust, provision of appropriate equipment or signposting to outside organisations.
