Specialist Occupational Therapist (Gedling & Hucknall IHTT) with Nottinghamshire Healthcare NHS Foundation Trust in Bulwell - Nottingham
The Mental Health Services for Older People's Gedling & Hucknall Intensive Home Treatments Team (IHTT) primary purpose is to enable people to live as independently as possible at a time when they are needing urgent and intensive support due to a mental health need. The team values high quality care, uses creativity and flexibility to provide a truly person-centred approach. IHTT work with people with a diagnosis of dementia or people aged 65+ with a mental health need, utilising their specialist skills and knowledge of mental and physical health to maximise outcomes for the person. IHTT recognises the importance of people recovering in their own home so work to prevent unnecessary admissions into hospital and residential care settings, and to facilitate a timely return home following an inpatient stay. The teams provide an urgent response to referrals within 24-48 hours and can support people in the community for up to 8 weeks. The teams work across 7 days a week . People referred to the IHTT can expect that a clinician will see the person at home to assess their needs and to work with them to construct a collaborative care plan that is individually tailored to their specific mental health needs. Over the course of the initial first weeks, the person will receive regular visits from other members of the team as per their care plan, and a weekly review by a clinician to assess the progress and make changes to the plan as required. When ready for discharge, the clinician will look at the person's ongoing needs to identify appropriate support and make any referrals needed to facilitate this. Throughout the time with the team, people can expect to be treated with dignity, compassion, and respect. Some of the duties of this job include: - To provide an occupational therapy service for older people with complex mental health needs- To carry out occupational therapy assessment, goal setting, implementation of treatment programmes and evaluation.- To establish the needs of individuals referred to the service and plan recovery focused care with colleagues from health, social services, families, and the voluntary sector.
