Team lead with Kent Community Health NHS Foundation Trust in Maidstone
We are looking for a Band 6 nurse to join one of our Community Nursing teams based in ABC PCN (Maidstone),to support the development of this service. The service operates seven days a week from 08:00 - 16:00, withoccasional late shifts of 10:00 - 18:00 on a rotational basis. Weekend working is generally on a 1:4 ratio withdays off during the week.The team support patients registered with GP practices working in the Maidstone town and outlying villageswithin the ABC Primary Care Network.Each team has two senior nurses to take responsibility for a defined geographical area, and provide clinicalleadership to their Band 5, Band 4 and Band 3 staff. The team will work together to manage caseload patientsdelivering a high-quality service. Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car leaseprovision, following their probationary period. Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self-management where possible, and improving patient lives.Together we can empower you to really make a difference to patients' lives. The very different world we areliving in at the moment has given us the opportunity to rethink how we provide our services here at KCHFT.Successful applicants are required to have the qualification level specified in the person specification in orderto carry out the job, however, we do welcome applicants from individuals wishing to further their knowledgeand skills and to gain this recognised qualification, which we will support.
