Bank B6 Specialist physiotherapist with Central London Community Health Trust in London
Long days. 12 and half hours shifts with 1 hour break. Either 8 till 20:30 or 10:00 till 22:30 To deliver a wide range of specialised physiotherapy treatment as part of the Unplanned Care services. This includes working with patients who have a range of complex needs to facilitate timely discharge from acute care and avoid admission. The service operates over 7 days a week between 8am and 6pm (flexible working/long days can be discussed). Staff will be required to work at weekends and varying shift patterns across core hours depending on service needs for the future. Be an active member of the clinical team, providing a comprehensive therapy service to residents of West Hertfordshire referred into the service. Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation. To work with patients and their carers or relatives in their own homes. Build and maintain key relationships with referrers to the service, to facilitate appropriate referrals and an understanding of the role of the Physiotherapist and the Multi-professional team Support self and service development through active participation in Continuing Professional Development (CPD) and by participating in any audit or research being undertaken by the service . Undertake specialised uni-professional and multi-professional assessments. To develop and implement treatment plans for clients. These clients will present with an exacerbation of a frailty related condition and will likely have Multiple co-morbidities including neurological conditions, musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis. Participate in the Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate; including the ordering of moving and handling equipment relevant to rehabilitation need. To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with Trust Guidelines for sharing of information and informed consent. These agencies include General Practitioner's, Social Services, Healthcare professionals, voluntary agencies, and residential homes. Be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. To be able to interpret and follow these guidelines in circumstances where clients do not have capacity to provide informed consent.
