PCN Cancer Facilitator with NHS West Yorkshire Integrated Care Board in Wakefield, West Yorkshire
An exciting opportunity has arisen for an experienced, highly motivated candidate to join the West Yorkshire and Harrogate Cancer Alliance to support the Health Inequalities programme; working with a wide range of stakeholders including primary care, secondary care, and voluntary sector to deliver timely presentation, early diagnosis, screening, primary care, and best practice timed pathway objectives. You will focus on reducing cancer-related health inequalities and will use data to demonstrate patient need, prioritise areas requiring support and measure impact, sharing these across the Alliance and wider. You will provide support to Primary Care Networks (PCNs) to improve the quality of their services, and work towards achieving cancer-related objectives as described in the contract ED DES and wider. You will also provide a vital communications channel between primary care, secondary care, and the Alliance work programmes. You will have demonstrable experience implementing evidence-based interventions. The candidate will bridge the gap between National Cancer Programme requirements and primary care, providing a crucial link between implementation and wider strategy. You will provide a key communication channel between the Cancer Alliance and primary care, facilitating the transfer of soft intelligence and identification of need, whilst simultaneously creating potential for primary care involvement in Alliance workstreams and ensuring that successful projects are promoted and shared across WY&H and wider. You will need to work flexibly to meet the needs of PCNs, agreeing several local objectives and aligning work with the health improvement agenda and Core20PLUS5 initiatives to prevent more cancers. This will include a mix of office- and home-based work with frequent travel e.g., to visit PCNs and attend meetings. The candidate will build and support the management of complex relationships between individuals and organisations and respond appropriately to challenges, seeking support and guidance as required. About us This post will be part of the West Yorkshire and Harrogate Cancer Alliance Programme Management Office. The Cancer Alliance works within the West Yorkshire Health and Care Partnership to drive delivery of the NHS Long Term Plan ambitions for cancer to improve survival and diagnose cancers earlier. The Alliance, as West Yorkshire & Harrogate system leaders for cancer, drives the change needed to achieve the ambitions, providing an opportunity for a different way of working to improve and transform cancer services. Their focus is also on leading transformations at scale to improve survival, early diagnosis, patient experience and long-term quality of life to patients and people affected by cancer. This post will support primary care professionals and practice teams across WY&H. You will be part of the West Yorkshire and Harrogate Cancer Alliance which works across the cancer pathway,and spans the whole WY ICB.
