Lead Cardiac Genetics CNS with King's College Hospital NHS Foundation Trust in London
The post holder will lead a team of specialist nurses across Kings Health Partners within an existing team of ICC clinicians and genetic counsellors. The successful candidate will provide professional and managerial supervision and leadership to the nursing team, as well as undertaking clinical responsibilities with ICC patients and their families. To act as the clinical lead in the provision of a multidisciplinary approach to the ICC service across all sites of KHP To act as an autonomous practitioner for clinical decision making, complex patient management, assessment and treatment for patients. To provide a Trust wide service that offers advice and support for all patients who require cardiac pedigree tracing, family support and the organising of genetic testing and counselling. To forge effective links with other relevant departments and specialists within the Hospital and CCG and wider community groups. To influence the strategic development of ICC service within the Trust, locally and across the South East Sector The post holder will develop new areas of practice with nursing and consultant staff to ensure quality standards are met. Setting up new pathways for patient treatment and follow-up About us To adhere to the NMC Code of Professional Conduct, working within and accepting responsibility for maintaining agreed levels of competence. To be fully conversant with local policies and guidelines and adhere to these at all times. To maintain him/herself in a professional manner, ensuring that ICC Service interfaces with all other departments in a professional and productive manner and acting as a role model to junior staff at all times. To maintain confidentiality with regard to information of a personal nature pertaining to both patients and staff. To participate in and assist with the operational issues involved in any research projects conducted within the department. To promote nursing research relevant to the speciality. To identify and allocate appropriate individual patient caseloads to the team and accept responsibility for own patient caseload, ensuring all patients have an appropriate, individualised and up to date plan of care at all times To act as an effective role model for best practice nursing in the field of ICC, acting as a specialist in the field and giving clinical advice as necessary and providing leadership for the team. To set standards of care, improving and developing these to ensure they incorporate current professional recommendations and evidence based practice. To agree and develop local policies and protocols, including those regarding nurse led admission and discharge to the service, to ensure the continuous delivery of a high quality and equity of access to the service. To be responsible for the management, supervision and deployment of the ICC nursing team, including allied health professionals and administrative staff and to lead in the process of selection, recruitment and retention of staff for the team. In conjunction with the Head of Nursing, provide direction for the service and ensure the achievement of identified goals, particularly in relation to the implementation of the National Plan. To ensure accurate and complete data collection with regard to the provision of secondary prevention and provide annual audit and data concerning cardiac rehabilitation, demonstrating the achievement of relevant standards set in the National Service Framework for Coronary Heart Disease. To empower and educate patients and their families in respect of self-care and sustainable lifestyle changes. To act as a resource for other health professionals on issues relating to cardiac rehabilitation and secondary prevention. Contribute to the development of nursing practice within the team, and work with the Genetic Counsellors to ensure a knowledgeable and skilled nursing team.
