The Learning Disability Intensive Support Service (LDISS) is a nationally recognised service, it provides people with a Learning Disability with behaviours that challenge the service or a mental health condition that cannot be supported in mainstream services, the required support in the community with the aim at keeping people at home and out of hospital. LDISS aims to give service users a meaningful, high quality and unrestrictive life where possible. Through providing the right interventions and support, service users will be assisted to live a life where they have choices, are included in their communities and achieve their potential. The LDISS Team, as part of the Gloucestershire Health and care NHS Foundation Trust provide a specialist service for people with a Learning disability whose behaviour is causing significant impact on the individual their family or the services supporting them. To be a member of a multidisciplinary team providing a service across 7 days a week. LDISS is operational 365 days year working 08:00 - 20:00 however occasional night shifts may be needed. Staff are expected to work flexibly across a 24 hour period when required. To work with both adults and children To maintain responsibility for a shared caseload of up to 30 service users. The post holder will be responsible for the implementation and collection of data required for clinical audits The job role spans both LDISS and inpatient services and delivery into these services will be based on priority need. The inpatient services provide specialist assessment and intervention on a short-term basis for those who require additional support and whose needs cannot be managed within their home environment. The qualification, training & experience requirements for the role are underlined in the Job Description/Person Specification. About us The post holder will be a senior member of the Learning disability Intensive Support Service (LDISS) working with service users throughout Gloucestershire. The purpose of this role is to: Provide specialist skills and knowledge to deliver evidence based occupational therapy assessment and treatment to service users with complex learning disabilities Work as a key member of a specialist multidisciplinary service contributing expert knowledge and outcomes of assessments to support the creation of specialist intervention plans to manage complex needs including behaviours that challenge others Provide advice, and training to support primary and secondary health care professionals, social care providers and carers to enable them to better meet the health needs of service users Optimise the service users ability to remain in the community and / or be cared for in the least restrictive environment. Facilitate appropriate discharge from hospital and respond to agencies requiring assessment of service users who are being considered for hospital admission. Implement and monitor PBS strategies or equivalent strategies. Be lead mentor for students under the remit of the profession Supervising staff at band 5 and below
