Frailty Clinical Nurse Specialist with Worcestershire Acute Hospitals NHS Trust in Worcester
We have an exciting opportunity for 2 candidates to join us in our expanding 7 day Frailty journey. This post will be working across Worcester Royal Hospital and Alexandra Hospital in Redditch. Shift patterns consist of long days 7:30am to 8:00pm with a 60 minute break. This post include weekend working. We are open to full time and part time positions. For an informal discussion please contact Matron Gincy John via switchboard or email gincyjohn@nhs.net To ensure cohesive working between Acute and Community Services To contribute to the organisation of frailty multidisciplinary, patient centred care To facilitate flow of patients both from admission via A&E/MAU and through the whole patient journey. To provide expert relevant clinical advice and support to other health professionals, patients and their carers To support the development and implementation of best practice within the speciality of frailty medicine To provide a specialist nursing role in the assessment and management of patients who are frail. To provide an outreach service to other patients in the Hospital who have a high frailty score To provide key support and an educational resource for any qualified and unqualified nursing staff, allied health professionals, junior doctors and students working within the speciality To develop nurse led frailty follow up telephone advice after discharge. To play a key role in ensuring that national and local targets for frailty patients are met.
