The Lawns Medical Practice located in Diss, are seeking an experienced and dedicated practice nurse to join our team on a full-time basis.We are a supportive and friendly practice with a manageable workload. Undertaking health assessments, administering medication and treatments. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Wound care.
