                                                     We are looking for an Advanced Clinical Practitioner (ACP) to work as part of a new team at Ranworth PCN.Ranworth PCN is part of the national PCN pilot looking at new ways of working in primary care. We welcome applications on a full and part-time basis.     In line with the Fuller report, theproactive ambulatory care model is part of a broader shift within the NHS toward more community-based, patient-centred care that aims to keep individuals out of hospitals and support them in managing their health conditions more effectively at home.
