Grovelands Medical Centre are looking for an enthusiastic Practice Clinical Pharmacist to work closely with our practice teams - to deliver our pharmacy vision, delivering excellent pharmacy-led services for patients registered with the practice. Experience in General Practice is desirable. The pharmacist will be expected to contribute to the leadership of medicines-related issues for the surgery, and to provide leadership and mentoring to any clinical pharmacists, pharmacy technicians and prescribing support staff employed across the Primary Care Network (PCN). Essential Requirements Registered with the GPHC Degree or equivalent in pharmacy Desirable Requirements Previous experience in primary care setting Clinical Diploma Independent prescribing qualifications Up-to-date clinical knowledge, a thorough understanding of primary care prescribing, medication reviews, experience of designing and implementing strategies for improving prescribing. You will be practicing at a senior level, have excellent written and verbal communication skills, experience of influencing others, conducting, and managing face to face and remote clinical caseloads in primary care. Undertake a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. Deal with the repeat prescription system, acute prescription requests, management of medicines on transfer of care: discharge letters, treatment advice notes and medicines reconciliation. Provide telephone clinics for patients with questions, queries, and concerns about their medicines. Perform structured medication reviews, manage some aspects of the quality and outcomes framework and enhanced services. Work with the practice in enabling them to develop and apply systems for safer prescribing. Develop good relationships between local community pharmacies, and the PCN pharmacy team. Develop and deliver medicines-related training for GP and ACP prescribers in the PCN, and education events for clinicians and patients. Conduct periodic meetings for identifying and reviewing prescribing and patient safety issues. Lead and own the Prescribing Quality Scheme, PINCER and QoF aspects for prescribing and audits. Work with care home staff to improve safety of medicines ordering and administration and conduct periodic care home visits to help achieve this. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
