Community Learning Disability Nurse - Development Post with Herefordshire and Worcestershire Health and Care NHS Trust in Bromsgrove, Worcestershire
An exciting opportunity has arisen to join the North Worcestershire Community Learning Disabilities Team as a Band 5 to 6 (developmental role) Community Learning Disability Nurse based at Princess of Wales Community Hospital, Stourbridge Road, Bromsgrove. The successful applicant will become part of a small but committed team who are passionate in ensuring that individuals with a learning disability receive equitable health care, alongside managing an effective caseload and maintaining regular visits to service users and their carers. The team is currently involved in exciting service developments, including RcPsych QNLD Accreditation, STOMP, Dementia pathways, and transitions pathways to better meet the needs of individuals with learning disabilities. Regular individual and group supervision and support will be provided as well as on-going CPD and training to help further develop your skills and competencies in this specialist field. Facilitate/teach effective systems to carers to enable the transfer of skills required for the management of service users healthcare needs. Facilitate Health Action Plans for each service user on caseload, to enable access to the full range of primary and secondary care services available. Promote and maintain good working relationships and systems of effective communication through liaison with service users, carers, and other professionals / agencies. Maintain accurate and up to date clinical records in line with Trust/NMC standards of practice. Record activity to meet the requirements of Patients Information Management System. Attend team and multi-professional meetings as appropriate. About us Ensure the process of referral, assessment, planning, intervention, and evaluation of care is efficiently and effectively delivered with clear quality outcomes. Promote the values and principles of person-centred planning approaches, maintaining effective continuity of care. Undertake specialist healthcare assessments, ensuring referral evaluation, data analysis and intervention planning is identified to enable service access. Implementation, monitoring, and evaluation of therapeutic nursing interventions based on a comprehensive assessment using a recognized nursing model of care. Encourage others to develop themselves and the service through improvement, innovation, and continuous development. Joint working with others, e.g. patients, colleagues in the delivery of high-quality healthcare. Build positive working relationships, respecting and valuing others, being helpful and inclusive.
