Sheringham Medical Practice are looking to welcome a dedicated and knowledgably clinical pharmacist into our forward-thinking and supportive practice to play a full and active role within our multi-disciplinary team. This exciting role is offered on a full-time basis. Our clinical pharmacist will play a vital role within our team, managing medicines-related queries, carrying out structured patient reviews, and contributing to our medicines optimisation agenda. With a well-trained administrative team skilled in managing clinical correspondence, data entry, signposting, social prescribing, and medicines management, our practice is focused on delivering high-quality care and support for both staff and patients. Being the primary contact for medicine-related queries. Conducting in-depth medicine reviews with patients. Performing structured reviews in care homes. Assessing and treating patients, guiding to healthcare services as required. Working closely with GPs on medication updates and solutions. Collaborating with the dispensary and practice teams to support medicine optimisation goals. Promoting cost-effective prescribing. Managing acute and repeat prescriptions. Conducting clinical audits.
