Women's Health PCN Care Coordinator with Jubilee Medical Centre in Liverpool
In 2020 North Liverpool PCN launched the first fully integrated inter-practice Long-Acting Contraception clinics for primary care in Liverpool. These clinics became the foundation for what are now known as the Liverpool Womens Health Hubs. The Liverpool model for Womens Health Hubs was the key case study published in the Womens Health Strategy and has become a flag-ship model for many other areas developing similar services. Since these services first launched, we have seen a 150% uptake in LARC procedures compared to pre-pandemic figures. Through the womens health hubs, we now can offer many women access to simple community gynaecology services such as implants & coils, pessary changes, menopause care, basic STI screening and cervical screening. The womens health hubs are also continually evolving, expanding and exploring additional services for our patients. North Liverpool PCN is looking for a highly organised, motivated individual to support the day-to-day running of our Womens Health Service and Supporting of the PCN. The post holder would need to be extremely efficient, professional & understand the importance of good access to high quality womens health care (especially in the current climate). We would expect an applicant to be very good at working in a team and have a passion to help develop our services. The post holder is expected to be a strong communicator and have excellent administrative skills. The successful candidate is expected to have an enthusiasm to train in new skills and to work alongside our GP lead for Womens Health to help develop and expand the Womens Health Hub and to also support the primary care network Care co-ordinator. The successful candidate will work 3 days as a womens health care coordinator and 2 days supporting the PCN. Managing appointments (via EMIS Web) Organising efficient day-to day running of the Womens Health Hub Contacting patients & clinicals over appointment schedules Liaising with practices & health care professionals Answering patient queries Managing waiting lists Managing stock requests Service promotion across the PCN Running EMIS searches Use of Excel programme to support LES claims Working closely with the Clinical Lead of the Womens Health Hub (WHH) to facilitate advancement & promotion of the services Manage PCN Email queries Manage Link worker and Mental health practitioner referrals Support PCN clinical leads with projects Support with Covid vaccination centre administration Support covid vaccinators in housebound patients/Care homes when needed About us North Liverpool PCN is looking for a highly organised, motivated individual to support the day-to-day running of our Womens Health Service and Supporting of the PCN.
