This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in South London and Maudsley (SLaM). The FIND Team will provide safe transition for inpatients discharged from the inpatient Forensic Services to the community providing specialist assessment, treatment and consultation to the Forensic Service in order to ensure a smooth, safe and clinically justified pathway to individuals with mental illness, offending history and learning disability with or without neurodevelopmental disorder. The three forensic services of SLaM, South West London and St Georges( SWLSTG), and Oxleas together comprise the South London Mental Health and Community Partnership (SLP). The SLP's overarching goals centre on bringing care closer to home, improving patient pathways and outcomes by delivering excellent care in better environments. The SLP has a track record of delivering highly effective, clinically informed service developments in order to better meet the needs of service-users. The FIND provision is one such development and the SLaM FIND Team will therefore work closely with the FIND Teams in each of the other Trusts in order to expedite service-user's transition through inpatient settings to community provision. Working within the scope of professional practice the post holder will provide a key role in leading and managing a specialist community mental health service. Using evidence based inclusive leadership skills to develop a service to provide excellent evidence-based interventions. The post holder will be expected to promote a culture of service user and carers involvement at every stage of the care pathway. The role will be key in monitoring clinical governance and ensuring that the service provides safe and quality services to people needing the support of the FIND Team across Croydon, Lambeth, Lewisham and Southwark. The team sits with the larger Community Pathway within the Croydon BDP directorate who oversee the Forensic Community Teams, Mental Health Learning Disability Teams and Transforming Care in Autism team. About us Ensure that all clinical practice is developed using up to date evidence base, ensuring that all staff have the appropriate skill base and supporting mechanisms to access training to work with the service user group. Oversee a service user's care pathway, utilising available resources and Patient Journey framework and lead the team in its implementation. Ensure systems are in place to support user and carer-centred involvement in the planning and provision of care. Ensure that staff receive regular supervision and appraisal, and are offered essential and developmental training in accordance with their personal development plans, and the needs of the service. Ensure that all service users are made aware of, encouraged, and assisted to use all services available to them. Ensure that the requirements of all mental health legislation are met with particular regard to the Mental Health Act 1983 (including amendments and Code of Practice) and Community Care Legislation (eg. CPA, Supervision Registers and Supervised Discharge etc), NSF, Child Protection and NHS Plan, Care Act 2014, and ensuring that all practices are within the guidelines set by the NMC and are in accordance with Trust policy. Ensure that the ACOMHS standards are upheld by managing an efficient referral system and supporting the delivery of the range of interventions recommended by the NICE guidance.
