The Childhood Immunisation & Health Inequality Lead role is an exciting opportunity that combines responsibilities from two key areas: increasing childhood immunisation coverage and addressing health inequalities within the our resident population. The role focuses on improving health outcomes for children under 5 through immunisation efforts while also working on broader health inequality projects that target disadvantaged populations within the Springfield Park PCN areas. This position requires a proactive approach to both clinical service delivery and community engagement, ensuring that underserved groups receive appropriate healthcare interventions. The role involves close collaboration with PCN staff, local healthcare providers, voluntary organizations, and community groups. About us Childhood Immunisation Responsibilities: Immunisation Delivery : Provide childhood immunisations for under-5s, including Flu, HPV, MEN ACWY, and Td/IPV, MMR in clinical settings and through outreach and domiciliary visits. Coordination : Work with practices to organize immunisation clinics, manage patient invitations, and coordinate care for specific patient cohorts, especially those hard to engage. Data Collection & Analysis : Gather and analyse data on immunisation rates, and contribute to developing strategies to improve uptake. Clinical Support : Adhere to infection control procedures, manage clinical incidents, and ensure proper communication and coordination with the broader healthcare team. Education & Training : Provide training to practice staff on immunisation protocols and support ongoing professional development. Health Inequality Responsibilities: Community Engagement : Identify and engage with communities experiencing health inequalities, particularly focusing on ethnic minorities, economically deprived groups, and those with chronic health conditions. Data-Driven Interventions : Utilise available data to identify health disparities within the PCN and design targeted interventions such as Des- CORE20PLUS5 Program Implementation : Lead the implementation of health inequality initiatives such as improving CVD management, increasing cancer screening uptake, and enhancing access to mental health services. Collaboration : Work with local system partners to address barriers to care, develop community assets, and create sustainable health improvement strategies. Evaluation & Reporting : Establish processes to evaluate the impact of health inequality interventions, report on progress, and adjust strategies as needed. To be responsible for ensuring community engagement work takes place ensuring aims and objectives are met. System level : to represent the general practice neighbourhood and community partners in the ICS inequalities improvement agenda. Place level : supporting the integration of primary care, secondary and community services, in collaboration with peer organisational HI Leads, whilst driving the change to help mitigate inequality. Engaging with other place- based partnerships. Neighbourhood / Community Level : to build collaborative, sustainable and positive relationships with external partners to develop a shared vision and understanding of the issues and challenges and how to work together to meet the needs of the community ensuring that it is from a population perspective. Key Relationships Internal : PCN staff, doctors, trainees, and administrative personnel. External : Local health authorities, voluntary sector organisations, community groups, patients, and other key stakeholders involved in addressing health inequalities.
