Community Mental Health Nurse - band 6 We have an exciting opportunity to offer the right person to be a part of our committed and enthusiastic team working with people who have complex needs relating to their diagnosis of dementia and mental health illness. Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England's new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people. This post will sit within our multidisciplinary community dementia team and will work as a lead practitioner to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home and able to maintain the relationships that are important to them. The service has an intensive support element within it that offers extended hours and weekend interventions to people who are seen by the team. The service operates 0900 - 1700 hours Monday to Friday Applicant must be able to drive and have access to a vehicle for work. They will demonstrate an understanding of clinicalinterventions relevant the care group to include education, care plans and discharge planning,working closely with carers, families and partner agencies to facilitate this. The post holder willwork at all times to promote the safety and the well being of service users and theirfamilies/carers. The post holder will assess, plan and implement care, and provide specialistnursing advice and carry out specialist nursing procedures. They will provide clinicalsupervision to staff and students. This post offers the Trust's £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions). About us Detailed job description and main responsibilities Clinical To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service. To support/manage a team caseload and attend relevant MDT meetings To regularly support and cover the duty rota To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway To provide risk screening and assessment as well as risk management planning To contribute to and facilitate evidence-based assessments and individual and group interventions To contribute to the maintenance and development of the Service To regularly share and supervise students on practice placement, and junior staff as appropriate
