Urgent Community Response - Rapid Response RN - Band 5 £34,521 - £41,956 Inc HCAS per annum pro rata Full time - 37.5 hours per week The post is based within the Rapid Response Team. This service provides a multi-disciplinary urgent assessment and crisis intervention service within a 2 hour response, and at times a 2-6 hours response to patients in the community with the aim of preventing hospital admission operating 7 days per week 8am to 8pm. Our service is looking for a proactive and enthusiastic staff nurse who is excited to be part of our team and who is willing/interested to progress their career. The post holder is a Nurse should have good community experience and is skilled to see patients independently, making autonomous decisions in the clinical and diagnostic assessment of patients with potentially complex health and social care needs to support keeping them in the community and reduce hospital admissions / ED attendance and to develop a comprehensive plan of care. The post holder will have the support of the Advanced Practitioner/GP where there is uncertainty or a need for additional support. The Referrals are accepted to service from GPs London Ambulance Service (LAS), care homes, and community providers. Proposed Interview date week commencing 25th November 2024 - Can be subject to change To comply with the duties placed on employees by the Bromley Healthcare's Health and Safety Policy and Related Procedures and to act in accordance with all instruction, information and training supplied in relation to those duties. Such other duties as requested, compatible with the post, which may arise from time to time and flexibility to support the services within UCR. Observe the confidentiality of records and health information at all times and be aware of and practice the principles of the Data Protection Act of 1984, as this applies to computerised information. To undertake appropriate training to meet individual developmental needs within the role. Undertake the measurement and evaluation of work and current practices as directed by team lead as part of the clinical audit programme. To work with the Rapid Response Lead /ANP /ACP to facilitate changes in practice across the service to improve clinical outcomes to meet the needs of patients and carers. To respond to urgent referrals into the service from GP/111/LAS/Care Homes and HCPs for patients to be seen at home, who have deteriorated below baseline due to illness or injury and will require additional clinical support, medication prescribed and care plans to prevent attendance to ED or hospital admission. To participate in achievement of service objectives to demonstrate productivity, performance and effectiveness About us To undertake a focused person-centered approach, ensuring quality standard and delivery of care for patients. To develop safe clinical skills in the community setting to proactively manage very high-intensity users of health and social care services, with multiple co-morbidities and pharmacy, social, medical and nursing needs and who are at risk of hospital admission. To undertake nursing procedures as indicated for example, catheterisation, monitoring of vital signs and review of symptoms, and phlebotomy. To proactively monitor and manage a patient caseload with sound clinical autonomous decisions about patient care and seeking support from colleagues as appropriate. To confer, as appropriate and work with the patient's GP, BHC GP/Consultant, Directorate Lead Nurse, Clinical Lead, ANP, ACP and/or Out of Hours GPs to ensure there is robust communication around clinical issues as indicated. To sensitively deal with distressing or emotional circumstances regarding patient care, and communicate information, sensitively, requiring empathy and reassurance, to patients, and carers. To actively seek GP/Frailty Consultant to review polypharmacy and other medication issues in accordance with the Department of Health, Professional and organisational guidelines. To work effectively as part of a multidisciplinary team working to support the common goal of safe, efficient care delivery closer to the patients homes. To empower patients and their families through appropriate support and education. To utilise formal and informal reporting mechanisms, including electronic and handwritten reporting to ensure effective internal and external communications and to ensure all documentation is accurate and professional. Fully support the use of EMIS mobile and provide contemporaneous clinical records. To continuously challenge practice ensuring a sound evidence base thus meeting clinical governance requirements. Promote multidisciplinary teams working to provide rapid crisis intervention, the common goal of safe efficient care to people in their own homes. Support the Service Lead and Operational Leads in delivering the UCR KPIs and ensure that capacity is maintained, and timelines managed at times of escalation. To contribute to partnership working with Acute Trust GPs, Nursing/Residential home staff and other health care professionals within Bromley Healthcare, Social Services and voluntary agencies to promote an anticipatory care planning
