An opportunity has arisen for an Advanced Clinical Practitioner to join our dedicated team based at Kings Park, supporting the community of Bournemouth and Christchurch.
We are looking for a desire to engage, motivate and innovate within this specialised and autonomous role. You will have advanced skills and be able to assess, diagnose and formulate a clinical management plan for the patients with complex, chronic illnesses or requiring end of life care.
Working together with Primary Care, Community Services, Social Services and the Voluntary Sector to provide enhanced and pro-active care and will be expected to act within a range of services delivered in the hubs including inpatient services, Virtual Wards, rapid access services and care closer to home.
Applicants should have a broad experience working within community or urgent care settings at a senior level and knowledge of working within the field of older peoples care. You must have proven experience of working across the pillars of Advanced Practice and are expected to have successfully completed the MSc Advanced Clinical Practitioner programme and be a non-medical prescriber.
We would be happy to accept applications from those who are nearing completion of their masters in Advanced Clinical Practice (3 months) those not yet completed would receive Band 7.

To provide high quality person centred care which always considers peoples' safety, privacy and dignity using advanced clinical knowledge that is evidence based.
As part of a multi-professional team, the post holder will practice autonomously, whilst being accountable and self directed in line with the relevant code of professional conduct. Plan and manage complete episodes of care; undertake independent comprehensive assessment and management of service users to incorporate pharmacological considerations; making complex clinical decisions regarding service usermanagement and clinical outcomes. To provide expert clinical advice and leadership ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision making skills.
To lead as a driver for change, to monitor and improve standards through supervision, evidence based practice, clinical audit, research and education. To work strategically, contributing to the planning and development of integrated services
We actively support learners, including those undertaking the ACP programme, there will be an expectation that the post holder will provide clinical supervision and support.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
