The service forms part of the wider corporate capacity, flow and palliative departments, directly reporting to the Deputy Chief Operating Officer, and primarily is focused on the safe discharge of patients. The discharge element of the service works closely with community colleagues on the Discharge to Assess model of care as well as supporting the management of patients with a long length of stay, patients requiring specialist discharge (End of life and fast track) and complex discharges. The service may overlap with other department functions for patient flow To deliver effective leadership and management of discharge services within Lincolnshire for the acute trust- including recruitment, career development, performance, disciplinary, absence management and work evaluation including professional registration. Work with Adult Social Care and other partner organisations to develop a shared vision and delivery of integrated services. Ensuring resources within the directorate are utilised effectively to achieve best value. To monitor and control delegated budgets, overseeing the setting of annual budgets and forecasts, procure supplies, identifying cost improvement programmes and efficiencies. Partaking in 1:1 supervision with line manager. Ensuring appraisals and mandatory training are up to date for self and direct reports.Uptake of essential training and PDPs. Ensure robust mechanisms are in place to manage sickness. To lead the management of investigations and take forward the learning outcomes, in respect of complaints. To utilise highly developed specialist knowledge and skills to support and develop staff, facilitating the identification of training needs and the formulation of personal development plans from the appraisal process. About us To provide 7-day cover for strategic planning, management and leadership across discharge and reablement services in the acute trust in partnership with Community Health and Adult Social Care. Ensuring services are developed and responsive to national and local guidance in accordance with the priorities of the Integrated Discharge Hub to meet the needs of local population. To work in partnership with other providers of services for adults to ensure continuity of provision and joined-up care in readiness for discharge. To provide line management to the discharge team, safe discharge and lead meetings on behalf of the lead nurse for discharge. To deploy the actions specific to National Discharge Policy and effective chairing, monitoring and actioning of long length of stays over 21 days and Reason to Reside criteria application. The Discharge Matron will have responsibility for the operational leadership and management of services for patients on Pathway 1, 2, 3, and out of area transfers as well as all complex discharges. The role will coordinate and support multi-disciplinary services including nurses, AHPs, medics, social workers, support workers and administrative staff. The role will work across partnerships in Lincolnshire as part of the integrated discharge team The individual will be highly motivated, have positive effective interpersonal and change management skills to support service improvement and transformation.
