The post holder will work with children and young people aged 5-18 who are in the care of the local authority. This role is 30 hours per week. The base will be the closest ECCH base to your home. To work for East Coast Community Healthcare CIC (ECCH) in the health service for Looked After Children which provides a service across ECCH, Norfolk and Suffolk and its partner organisations in line with statutory national and local guidance and recommendations including Care Matters. Care Matters: Transforming the Lives of Children and Young People in Care, Promoting the Health of Looked After Children To complete statutory health assessments with children, young people and their carers for children, young people aged 5-18 who are in the care of the local authority. To promote and improve the health of Looked After Children by providing direct services and enhancing local co-ordination and communication between young people, carers, health, education, social services and voluntary agencies. To assess and formulate the health needs of children and young people Looked After, including physical and emotional aspects, cultural diversity, and health-promoting activities. About us Following national and local guidelines, plan, organise, deliver, and evaluate comprehensive health assessments to assess and formulate the complex needs of a looked after child or young person using evidence-based knowledge to develop health plans toward improving the long-term health outcomes for looked after children Empower children and young people to take responsibility for their own health and wellbeing The post holder needs to be an autonomous practitioner who can develop and embrace a flexible range of working regarding the way services can be delivered thereby ensuring achievement of positive outcomes for Looked After Children and Young People To work as part of a small team in further developing the LAC Specialist service, whilst maintaining and developing relationships within Health and partner agencies. Ensure that the health of Looked After Children is positively promoted, encouraged, and supported. Work closely with all professionals involved to ensure a comprehensive health assessment for children and young people is undertaken Develop and undertake age-appropriate health promotion advice and act as a resource providing health advice to children, carers, and professionals Establish robust lines of communication relating to health care between service users, professionals, and agencies To attend as appropriate reviews for Looked After Children to inform and advise on the child or young persons health needs Ensure those children who are Looked After by Norfolk or Suffolk Local Authority but are placed out of County have their health needs met in a timely and consistent manner To establish and maintain effective links across organisational and inter-agency boundaries To provide professional, advice and training on issues relating to the health of Looked After Children to carers and other agencies To model a good standard of health assessments or report writing for Looked After Children and to support other health staff in undertaking these To communicate and receive highly complex and sensitive information in relation to Looked After Children Compile reports for looked after children reviews or conferences and store and retrieve these documents electronically All roles within East Coast Community Healthcare CIC (ECCH) require staff to demonstrate our Values and Signature Behaviours in the care and service they provide to patients, service users, stakeholders and colleagues.
