About us JOB DESCRIPTION Job title: Practice Nurse Hours of Employment: 30 hours per week (Mondays, Tuesdays & Fridays) Responsible to: GP Partners Accountable to: HR Lead Interview Date: 19/11/2024 Job Responsibilities: Clinical Responsibilities: Chronic Disease Management: Provide comprehensive care and management for chronic conditions such as asthma, COPD, and diabetes, including regular monitoring, patient education, and collaboration with multidisciplinary healthcare teams to optimise patient outcomes. Conduct Peak Flow Readings and Spirometry as required. Monitor and manage patients with hypertension, offering evidence-based interventions to reduce cardiovascular risks. Travel Health: Deliver a holistic approach to travel health by advising patients on vaccinations, medications, safe sex practices, food hygiene, and sun protection to ensure safe and informed travel. Vaccinations: Administer childhood and adult vaccinations as per patient group directions, providing informed advice and ensuring adherence to immunisation protocols. Well Person Clinics & Health Promotion: Lead well person clinics and health promotion initiatives, offering lifestyle assessments and advice on diet, smoking cessation, alcohol reduction, and physical activity to enhance overall health and well-being. Baby Clinic Care: Administer vaccinations and offer comprehensive parental guidance and support. Cervical Cytology: Conduct cervical cytology smear tests according to local and national guidelines, ensuring accurate documentation and patient care. Venepuncture: Carry out venepuncture procedures following local protocols to facilitate diagnostic testing and patient care. Wound Management: Provide a holistic approach to wound management, including Wound Care 'Tier 2' lower limb, (ulcer care & Doppler) and implement care in line with current evidence-based guideline. ECG Monitoring: Conduct electrocardiograms (ECGs) to support the diagnosis and monitoring of cardiac conditions. Contraception Services: Provide advice and administer contraception options, ensuring patient education and adherence to clinical guidelines. Suture Removal: Safely and efficiently remove sutures, ensuring adherence to clinical protocols and patient comfort. Safeguarding: Maintain awareness of statutory and local clinical protection procedures, including referral systems. Possess the ability to recognise signs and symptoms of child abuse, ensuring appropriate actions are taken to safeguard vulnerable patients. Health & Safety and Compliance: Sharps and Waste Disposal: Safely manage the disposal of sharps and hazardous materials in compliance with health and safety regulations, ensuring a clean and safe environment for both staff and patients. Infection Control: Strictly follow infection control protocols to prevent the spread of infections and maintain a safe healthcare environment, adhering to all national and local guidelines. Emergency Management: Recognise and manage anaphylaxis in accordance with current UK guidelines, providing prompt and effective treatment during emergencies. Perform cardiopulmonary resuscitation (CPR) as required, ensuring timely intervention in life-threatening situations. Health & Safety Protocols: Follow established procedures for handling and cleaning body fluid spills, maintaining a safe and sanitary environment in line with health and safety standards. Administrative Responsibilities: Informed Consent: Obtain and document informed consent (verbal or written) in accordance with legal and ethical standards. Stock and Equipment Management: Monitor and manage the maintenance of stock and equipment, including refrigeration and emergency equipment, ensuring that all resources are readily available and in good working order. Vaccine Management: Ensure the effective management of vaccine storage, including maintaining the cold chain and proper stock rotation, to uphold the integrity of all immunisation products. Confidentiality: Maintain the highest standards of confidentiality regarding all patient and practice information, ensuring compliance with privacy regulations and safeguarding sensitive data. Documentation: Ensure all consultations are documented in accordance with Nursing and Midwifery Council (NMC) guidelines, maintaining accurate and up-to-date patient records. Identifying and managing risks associated with clinical and patient care processes and reporting potential risks identified. Proactively utilising training to maintain and update skills and leading the training of others on infection control and patient care procedures. Monitoring and maintaining infection control standards within the practice's facilities and equipment, including hand hygiene provisions and cleanliness. Managing safe sharps handling procedures, including staff training, proper use, storage, and disposal. Maintaining a tidy, clean, and safe working environment through adherence to infection control procedures, taking corrective action or escalating issues when needed. Actively identifying and addressing health and safety hazards, reporting them promptly and implementing corrective actions. Ensuring cleanliness and sterility in both personal and patient areas and maintaining high standards of cleanliness throughout the practice in collaboration with other managers. Participating in regular infection control training, at least twice annually. Overseeing waste management, including proper segregation, container management, storage, and collection procedures. Managing and training staff on spillage control and decontamination procedures, including equipment maintenance. Maintaining sterile environments throughout the practice.
