We are looking for an enthusiastic professional who can help the Community Independence Service (CIS) make Hammersmith & Fulham (H&F) a great place to grow old. As a member of the CIS Home First team, you will be expected to be a clinician with a broad range of competencies who can adapt to a wide variety of clinical circumstances. You will use evidence-based principles, current best practices, and specialist skills to assess, plan, implement, and evaluate interventions and to optimize patient safety following their discharge from the hospital. You will deliver skilled physiotherapy or occupational therapy care to clients in their own homes, based on a thorough holistic and risk assessment of their clinical and personal needs and the agreement of an individualized care plan. You will enjoy the rewards of working with a client group for whom you can make a significant difference every day through your face-to-face client visits and care plan management. You will experience the strong motivation of being part of a supportive multi-agency effort that includes medical, mental health, nursing, allied health professionals, social care, and reablement staff. We will equip you for mobile working and actively support flexible working practices. The service is operational 7 days a week 8 am to 8 pm. Hours of work for therapists will vary between these hours. The position is 9 months Secondment position. o To support the transition of patients from acute services to the community and provide community assessments for people in the home first pathway.o The post holder will work in partnership with colleagues in the acute sector, social services, intermediate and primary care services to ensure that appropriate patients from the acute setting are transferred in a safe, timely and seamless manner into the community.o The post holder will coordinate the home first HCA care delivery.o The post holder will ensure effective onward transfer of patient care from home first to community pathways as required.o To identify/case find and pull potential CIS patients in partnership with acute colleagues ensuring that as soon as patients are medically stable they are transferred to the appropriate community service.o To independently undertake assessments of health and social care needs at home and plan community services required.o To support and enable the process of safe discharge from secondary to Community Services and assist in bridging the gap from hospital to home.o To proactively promote a seamless and multidisciplinary approach to health and social care provision.o To organise the overall daily management of the Home First team in order to ensure that service requirements are met, and ensure access, equipment and resources are in one place to allow the Health Care Assistant (HCA) team to safely meet the needs of the patient in the community.
