Senior Nurse Practitioner with Mersey Care NHS Foundation Trust in Huyton
We have an exciting vacancy for an experienced Registered Mental Health Nurse Band 6 to join our new Community Enhanced Rehabilitation Team (CERT). You will be joining a small but expanding multidisciplinary team who will provide high intensive community care for people with severe mental health problems, whom have been or would have been cared for in hospital rehabilitation placements. You will be based in Capper Grove in Huyton, Liverpool. We are looking for an applicant who is passionate about community rehabilitation to join our expanding multidisciplinary team (MDT). You will join other mental health nurses, a social worker, an occupational therapist, psychiatrist and clinical psychologist. We are also recruiting a support worker and peer support worker to make sure we work as a full MDT. The CERT team takes a formulation needs approach to a person's problems and difficulties and taking a holistic view on recovery. Helping people we work with move towards their own personalise goals. Like all team members you will have a caseload and be a keyworker for people with severe mental health problems. Training and comprehensive supervision will be provided. The post holder will support the Clinical Team Manager in carrying continuing professional andmanagerial responsibility for their clinical area. They will also contribute to maintenance of theoverall standards, which will include assessment of need and delivery of care within the contextof Effective Care Coordination. The post holder will work within an integrated team to ensure continuity of care for serviceusers on their journey through the acute service. The post holder will work from within the community environment but be expected to be flexible in terms of delivering care within avariety of settings About us Assist in the development and implementation of integrated care pathways, including appropriate service user referrals and models of care.Participate in case supervision to monitor named primary nurses standards of service user care and documentation. Give/receive regular clinical supervision as per Trust policy. Ensure the clinical environment is conducive to promote service users health, safety and well-being, thereby preserving and respecting each service users personal dignity, privacy, religious and cultural beliefs. Ensure record keeping is in accordance with Professional and Trust standards. Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code. Work in a professional and collaborative manner with the Multi-disciplinary team. Participate and promote positive intra-agency working. Participate in the referral process, including the assessment of such individuals in accordance with the agreed care pathway and the completion of quality reports where appropriate. Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care without supervision and act as Care Coordinator as required. Participate and assist in setting standards of service user care based on sound research findings.
