An exciting opportunity has arisen for Tissue Viability Specialist Nurse post within the South Tyneside and Sunderland Tissue Viability service. The service provides wound care advice, support, assessments, education and training to both hospital sites, community nursing teams, hospices, practice nurses, GPs, residential and nursing homes. The Trust aims to be recognised as one of the safest healthcare organisations both nationally and internationally. Reducing the incidence of pressure ulcers which have developed in our care is one of our patient safety priorities within our Quality Strategy (2018-2025). In addition wound management is a widespread and often underestimated health problem in the UK and the effective management of wounds has been determined a national priority with studies indicating that wound prevalence is set to increase due to increasing aging population over the next 10 years. Working within the Tissue Viability Team, you will play a key role in providing specialist clinical advice and support to patients and clinical staff in relation to pressure ulcer prevention and wound management. The tissue viability agenda is extensive and dynamic. This is an exciting opportunity for someone to join this established team and develop their clinical skills within this specialist field of nursing. You will work under the direction and supervision of the Senior Specialist Nurses, Lead Nurse and Nurse Consultant within the Tissue Viability team to provide Tissue Viability care, advice, support, education and training to patients, carers and staff. This is based on knowledge of relevant evidence, current research, best practice guidelines and tissue viability policies across the Trust. The successful applicant will have a passion for improving patient care, enjoy project work and understand the importance of good communication and evidence-based practice to improve patient safety, experience and clinical effectiveness. Applicants must have a sound working knowledge and recent experience of general aspects of tissue viability. Completion of a tissue viability course or associated wound care course is essential. Demonstrate leadership, contributing towards a culture of patient safety and continuous improvement in line with the clinical governance framework. Be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care within the specialism Provide specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area Take a lead role in the development of nursing staff in the field of tissue viability, through education and training. Play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions. Participate in research relevant to the needs of specialist area. Participate in service development within tissue viability across the Trust. Contribute towards the achievement of national and local standards/targets. Comply with the Trusts vision, values and behavioural compact.
